Your advanced course in dizziness and vestibular therapy

Benign paroxysmal positional vertigo (BPPV) is the most common form of peripheral vertigo and at the same time one of the most underestimated. While typical cases are usually recognized quickly and treated successfully, rare variants, atypical nystagmus, and persistent courses regularly present major challenges for therapists

Misdiagnoses and ineffective treatment approaches are unfortunately not uncommon in these patients – resulting in persistent symptoms and increasing uncertainty. This makes a deeper understanding of the pathophysiology, as well as a systematic and differentiated approach to diagnosis and therapy, all the more important.

This two-day advanced course is aimed at therapists with experience in vestibular rehabilitation who wish to expand their expertise in BPPV and specifically enhance their diagnostic confidence as well as their therapeutic skills in complex cases.

The lectures by Dr. Andrea Castellucci are delivered live via video conference. The content of his lectures is further reinforced on site by Firat Kesgin through brief comprehension checks and practical exercises.

Day 1 – Anatomy, physiology, diagnostic systematics, and modified repositioning maneuvers

with Firat Kesgin

  • Functional anatomy of the vestibular organ based on high-resolution MRI and microscopy images
  • Physiology of hair cells: movement of the stereocilia and kinetics toward the kinocilium
  • Semicircular canal–extraocular muscle coupling: neurophysiological foundations of nystagmus
  • Advanced diagnostic procedures and modified maneuvers: Tumarkin-like phenomenon, Loaded Dix–Hallpike test, Semont Plus maneuver, Li maneuver, and modified Yacovino maneuver
  • Lateralisation in the horizontal semicircular canal: lying down nystagmus, bow & lean test, and pseudo-spontaneous nystagmus
  • Central vestibular positional nystagmus: clinical presentation and differentiation from BPPV

Day 2 – Special forms and practical application of benign paroxysmal positional vertigo (BPPV)

  • Minimal Stimulation Testing: a new examination method with minimal vertigo provocation
  • Atypical posterior canal BPPV: apogeotropic form and involvement of the "short arm"
  • Atypischer horizontaler Bogengang-BPLS: Light Cupula, Heavy Cupula sowie Canalith Jam

  • Since 2020: Head of the Neuro-otology Outpatient Clinic at Santa Maria Nuova Hospital – in close collaboration with the Emergency Department and Neurology for the treatment of peripheral and central vestibular syndromes.
  • Since 2021: Member of the Scientific Advisory Board of VIS (Italian Society of Vestibology).
  • Since 2022: Member of the Bárány Society
  • Author of numerous peer-reviewed articles and book chapters; reviewer for otolaryngology and neurology journals; and guest editor of special issues in scientific journals.
